A health service for people over 18 years of age. Staff consists of board
certified Internists, Internal medicine residents of Michigan State University,
and nurses. Social workers, dietitians, pharmacists and physical therapists
are available upon request. Each patient will be assigned one physician
responsible for overseeing the patient's care.
Eligibility:
Over age 18. There is generally a four-week waiting period and a complete
physical examination is required on patient's first visit.

services; nutritional counseling; mental health interventions and health
education services.

charge to those patients who qualify. Accepts Medicaid and Medicare at

at VA facilities will be made as needed.

Service:
The Geriatric Assessment Center provides a comprehensive, multi-disciplinary
assessment for individuals over age 65. A physician, clinical nurse practitioner
or physican assistant and social worker perform the assessment. This is
a 4 1/2 hour assessment. Additional tests such as blood work, x-rays,
MRI or cat scan may be scheduled. After the assessment is complete and
all reports received a family conference is held so the physician and
social worker can discuss all results of the assessment and recommendations
for continuing care. A summary of the assessment results is sent to the
patient's primary care physician.
Eligibility:
Individuals over age 65 or those at risk of premature or inappropriate
symptoms. The symptoms may include the following: confusion, memory loss,
recent change in functions, multiple problems complicated by dementia
or psychiatric problems, immobility, falls, incontinence.
Cost:
Medicare, Medicaid, Blue Cross and other insurances are billed. There
is a $350 non-covered copay for the assessment.
