SELF DETERMINATION PROGRAM

 

Tri-County Office on Aging is one of four Pioneer Sites offering Self Determination in the state of Michigan . This program is expected to be offered state wide by the end of 2007.

 

WHAT IS SELF DETERMINATION?

Self Determination is based on the belief that you have the freedom to define your life, make meaningful choices regarding your life and the support you receive, and direct the services and supports you need to pursue that life. Self Determination is an option for people receiving or eligible to receive home and community-based services under the MI Choice Waiver (Project Choices). Instead of an agency overseeing your care, you receive a monthly budget, which you direct and can use to purchase care in a plan that works for you.

 

WHO IS ELIGIBLE?

Self Determination is available to anyone 18 years of age or older who meets the MI Choice Waiver (Project Choices) eligibility, regardless of their clinical condition or ability to direct services.

 

PRINCIPLES OF SELF DETERMINATION

•  Freedom : You plan a life with necessary supports

•  Authority : You control budgeted dollars, representing the cost of services in your plan, to purchase supports.

•  Support : You arrange resources and persons, both formal and informal, so you can live in and connect with your community.

•  Responsibility : You accept a valued role in the community and agree to use public dollars wisely.
