Services: A health service for people over 18 years of age. Staff consists of board certified Internists, Internal medicine residents of Michigan State University, and nurses. Social workers, dietitians, pharmacists and physical therapists are available upon request. Each patient will be assigned one physician responsible for overseeing the patient's care.

Eligibility: Over age 18. There is generally a four-week waiting period and a complete physical examination is required on patient's first visit.

Cost: Most insurance accepted.

Eligibility:  Individuals in need of medical services that do not have insurance or sufficient financial resources to meet those needs. Must sign up.

Cost: No charge to those who qualify. Also accept Medicaid and Medicare.

Services: Preventative health service and problem oriented health services. Provides primary care for children and adults, family planning, health counseling and immunizations. Cost: Sliding scale fee based on income and family size. Accepts Medicaid, Medicare, IHP and some private insurance. Donations welcome.

Services: Serving the health care needs of veterans in our community. Works in partnership with the Department of Veterans Affairs and Ingham Health Plan. Committed to providing quality primary healthcare in an efficient, professional and confidential manner . Staffed by nurse practioners. Services include laboratory, x-ray and prescriptions. Referrals to specialists at VA facilities will be made as needed.

Eligibility: Veterans with an honorable discharge

Cost: Varies depending on income and/or insurance coverage. Accepts Medicare, Medicaid, Blue Cross/Blue Shield of MI, PHP, Tricare & others.

Services: Include physical examination, ongoing medical care (short-term or long-term) and, as needed, mental health evaluation and treatment and coordination of referrals to community services.

Eligibility: Age 65 or over; new to Lansing area or in need of a primary care physician; persons with complex medical or psychosocial problems.

Cost: Medicare, Medicaid and Blue Cross accepted. Fee for service where applicable.


SPARROW SENIOR HEALTH/GERIATRIC ASSESSMENT CENTER

Hours: 8:00 a.m.- 5:00 Monday through Thursday. Friday 8:00 a.m.- 4 p.m.

Service: The Geriatric Assessment Center provides a comprehensive, multi-disciplinary assessment for individuals over age 65. A physician, nurse practitioner or physician assistant , social worker perform the assessment. This is a 4 1/2 hour visit. Additional tests such as blood work, x-rays, MRI or cat scan may be scheduled. After the assessment is complete and all reports received, a family conference is held so the physician and social worker can discuss all results of the assessment and recommendations for continuing care. A summary of the assessment results is sent to the patient's primary care physician.

Eligibility: Individuals over age 65 or those at risk of premature or inappropriate symptoms. The symptoms may include the following: confusion, memory loss, recent change in function, multiple problems complicated by dementia or psychiatric problems, immobility, falls, incontinence.

Cost: Medicare, Medicaid, Blue Cross and other insurances are billed. There is a $350 non-covered copay for the assessment.




 

 

Who we serve

We serve older adults primarily age 60 and older, and adults with disabilities.  We serve many people at low or no cost or on a sliding fee scale.

Nutrition

Meals On Wheels provides a hot, well-balanced meals delivered daily by friendly volunteers.  Senior Dining Sites are located throughout the tri-county area and serve those who are not homebound.

Advocacy

Learn about the issues that affect older adults and how to effectively use many communication tools to contact your legislators.

Information

Find helpful information on topics related to older adults and persons with disabilities including Social Security, health insurance, in-home services, Medicare/Medicaid, tax assistance and housing information.
